New patients may save considerable time by printing, completing and faxing the New Patient Appointment Form. Keep in mind that your doctor has referred you to me for my opinion about your condition and the potential need for surgery. I need to see ALL of your images (X-ray, CT, MRI, Myelogram, etc), tests (EMG, NCV, EEG, SSEP, labs, etc) and medications.
You may print and complete the New Patient Questionnaire before you arrive or when you get to the office.
